Patient Rights
Confidentiality
XRS is committed to maintain the highest standard in maintaining patient confidentiality rights.
We understand that patient medical information is confidential and we are committed to protecting it in accordance with all applicable federal, state and local laws. When we provide services to a patient, we create a record of the care and services provided. This record is necessary for us to provide the patient with quality care and to comply with certain legal requirements.
Any and all requests for patient medical information to be shared must be accompanied with all of the proper release forms necessary, signed by the patient or an individual with proper power of attorney documents showing authorizations. This verification is mandatory prior to XRS releasing any medical information.
